How to calculate daily vitamin D requirement (EN): vitamin D (cholecalciferol D3 from animal sources and skin synthesis, ergocalciferol D2 from fungal sources) is a fat-soluble vitamin and steroid hormone involved in calcium homeostasis, bone mineralization, immune modulation, fall prevention in elderly. Deficiency is endemic even in sunny countries: NHANES CDC USA reports 40-50% prevalence in US adult population <20 ng/mL 25(OH)D. The calculator implements RDA NIH IOM with correction factors for sun exposure, skin type and special conditions. Fitzpatrick skin type: light (I-II) 1.0x; medium (III) 1.2x; dark (IV-VI) 1.5x (melanin reduces cutaneous D3 synthesis from 7-dehydrocholesterol). Target serum 25(OH)D: 30-50 ng/mL (75-125 nmol/L) Endocrine Society 2011; IOM more permissive 20-50 ng/mL (50-125 nmol/L). Deficiency: <20 ng/mL severe, 20-29 ng/mL insufficient, 30-50 ng/mL optimal, >100 ng/mL (250 nmol/L) potential hypercalcemic toxicity. Conversion: 1 microg = 40 IU; 1 nmol/L = 0.4 ng/mL.
History vitamin D from Mellanby 1922 to Holick 1971
Vitamin D was discovered as rickets treatment in early 20th century after decades of observations that cod liver oil and sunlight prevented its onset in industrial city children (Glasgow England).
Edward Mellanby 1922 Sheffield UK identified a fat-soluble substance in cod liver oil that cured rickets in puppies (Lancet 1919; J Physiol 52:11 1922).
Elmer McCollum 1922 Johns Hopkins distinguished vitamin A from vitamin D.
Adolf Windaus University of Gottingen received Nobel Prize Chemistry 1928 for sterol structure work and vitamin D2 ergocalciferol from UV irradiation of ergosterol from fungi (Goldblatt-Soames 1923 milk fortification onset USA to eradicate rickets, mandatory in many states 1933).
Michael Holick Boston University Medical Center 1971 identified 1,25-dihydroxyvitamin D3 calcitriol active hormone (DeLuca-Holick Wisconsin 1971 Biochemistry 10:2799).
Anthony Norman University of California Riverside characterized VDR receptor and non-classical effects.
Roger Bouillon KU Leuven Belgium European leader.
Endocrine Society 2011 Holick et al guidelines recommended target 30-50 ng/mL and active supplementation.
IOM Institute of Medicine 2011 RDA more conservative 600-800 IU with target 20+ ng/mL.
NHANES CDC 1988-2008 cohort tracking vitamin D status US population; 40-50% adults <20 ng/mL.
USDA Dietary Guidelines for Americans 2020-2025 identify vitamin D as "nutrient of public health concern".
FDA Nutrition Facts label since 2016 mandatory display.

5 numerical vitamin D examples
Example 1 adult 35 yrs light skin low exposure: RDA 600 IU; sun factor 1.0x; skin factor 1.0x; supplementation 600 IU/day in winter (Dec-Mar northern US Boston). Target 25(OH)D 30-50 ng/mL.
Example 2 elderly woman 75 yrs medium skin low exposure: RDA 800 IU base * 1.0 sun * 1.2 skin = 960 IU/day; recommended 1000-2000 IU/day for fall and fracture prevention; NOF AACE recommend supplementation in osteoporotic elderly.
Example 3 pregnancy 30 yrs dark skin winter: RDA 600 IU base * 1.0 sun * 1.5 skin = 900 IU/day; ACOG and Endocrine Society recommend preconception and pregnancy supplementation 1000-2000 IU/day to prevent neonatal hypocalcemia tetany seizures and congenital rickets; Q1 25(OH)D check.
Example 4 obese BMI 35 sedentary winter: RDA 600 IU doubled to 1200 IU for adipose sequestration; * 1.0 sun * 1.0 light skin = 1200 IU/day; possible loading 50000 IU/week x 8 weeks initial in deficiency to fill adipose stores.
Example 5 known deficiency 25(OH)D 12 ng/mL: RDA 2000 IU base; loading phase 50000 IU/week x 8 weeks (= 400000 IU total), then maintenance 1000-2000 IU/day to target 30-50 ng/mL; recheck 3 months; Endocrine Society 2011 Holick protocol.
Vitamin D model limitations
Individual variability high: cutaneous synthesis depends on latitude, season, time of day (noon UVB peak), exposure (face arms vs full body), sunscreens (SPF 15+ blocks 99% UVB), air pollution, glass (UVB does not pass through car/office glass).
Latitude: above 40 deg N (Boston, Chicago, NYC, Madrid) cutaneous synthesis is null Nov-Mar; above 50 deg N (Seattle, London, Toronto, Berlin) endemic deficiency.
Age: skin over 70 yrs produces 4x less D3 than 20-yr-old (loss of cutaneous 7-dehydrocholesterol).
Pathologies: malabsorption (celiac, Crohn, gastrectomy, bariatric bypass), CKD chronic kidney disease (renal 1-alpha-hydroxylase deficit), liver disease (hepatic 25-hydroxylase deficit), hypoparathyroidism.
Drug interactions: anticonvulsants (phenytoin, phenobarbital, carbamazepine), chronic corticosteroids, rifampin, HIV antiretrovirals, cholestyramine, orlistat.
Acute D toxicity: >100 ng/mL 25(OH)D associated with hypercalcemia, hypercalciuria, kidney stones, vascular calcifications; mortality increased over 60 ng/mL some studies (U-curve).
UL Upper Limit IOM: 4000 IU/day adults; but in deficiency with medical supervision pharmacologic doses 50000 IU/week.
Measurement: total 25(OH)D (D2+D3) gold standard DiaSorin Liaison; LC-MS/MS Roche more accurate but expensive; inter-method differences +-20%.
VDSP Vitamin D Standardization Program NIH harmonization.
Common vitamin D errors
Common errors: supplementing without measuring 25(OH)D (VDSP Vitamin D Standardization Program recommends pre/post measurement); believing 10-15 minutes of sun is enough in winter at latitudes above 40 deg N (synthesis is null due to insufficient UVB); using SPF 30+ sunscreen all day in winter (reduces synthesis to zero); confusing 25(OH)D vitamin storage form (correlates with vitamin status) with 1,25(OH)2D calcitriol active (PTH-regulated, NOT useful for vitamin status); expecting fast normalization (8-12 week therapy for store reload; months-years maintenance); supplementing in hypercalcemia (check calcemia first); ignoring obesity (adipose sequestration requires 2x dose); using vitamin D2 ergocalciferol alone (D3 cholecalciferol more effective, 87% bioavailability vs 30-50% D2 Tripkovic 2012 meta-analysis); not considering interfering drugs anticonvulsants corticosteroids rifampin. Confusing units: 1 microg = 40 IU; 1 nmol/L = 0.4 ng/mL (frequent factor 2.5 errors).
